If equipped with spare tire lock, attach key to crank. Open rear cargo doors. If a cover plate covers the hold for the spare tire winch, remove the cover plate. Insert crank into hole until it engages the winch, then turn counterclockwise. If it won't turn: The winch is broken (usually caused by corrosion.) Cut the cable holding the spare tire up to get the tire out of the way, then install a new spare tire winch.
The spare tire is under the center of the van. The winch access is between the front seats.
Under the rear of the vehicle
To lower the spare tire you need to some tools and a jack. There is a cable winch holding the spare in place, so you will have to loosen it using your tools.
Remove the four 13mm bolts from the winch bracket and the whole assembly will come down.
It should be suspended underneath the rear center of the boxbed by a winch. Use your tire iron to crank the winch up or down by inserting it through the hole in the bumper and engaging the winch crank's socket. Turning clockwise raises the tire and counter-clockwise lowers it. Once the tire is on the ground and there is a bit of slack in the cable, just manoeuver the T-hanger through the center hole of the spare tire to release it. Do the opposite to rehang a tire.

i HAD THE SAME PROBLEM ON THE SIDE OF THE ROAD WITH A BLOW-OUT.I HAD TO TAKE A LONG EXTENSION WITH A SOCKET AND GO BETWEEN THE HOLES IN MY SPARE RIM AND REMOVE THE ENTIRE WINCH ASSEMBLY TO GET THE TIRE OUT FROM UNDER THE TRUCK.IT TURNS OUT THE INSIDE OF THE WINCH ASSEMBLY GOT RUSTY AND WAS KEEPING IT FROM RELEASING THE TIRE FROM THE WINCH.IT IS A PAIN TO GET OFF,I ENDED UP BEATING THE WINCH ASSEMBLY FROM THE BACK SIDE OF MY SPARE RIM WITH A 2 LB SLEDGE TO GET IT TO RELEASE,AND HAD TO REPLACE THE ENTIRE WINCH.GOOD LUCK.HOPE THIS HELPS.ON A GOOD NOTE,THERE IS ONLY 1 BOLT HOLDING THE ENTIRE ASSEMBLE TO THE TRUCK BEHIND THE SPARE TIRE,AND IS FAIRLY EASY TO REMOVE WITH A SOCKET AND LONG EXTENSION.
Use your jack handle. Side it through the hole in the body just below the tailgate. Guide the end of the handle into the winch assembly. Crank the handle to winch the tire down.
1. Remove the lock cylinder with key. 2. Pinch open the plastic retainer collar that connects the guide tube to the winch, and slide the tube out of the way. 3. Remove the 5/8" mounting bolt that holds the winch to the body. I needed a breaker bar to get mine loose. The spare tire on a 2008 Town and Country is located under the center of the van. There is an access between the front seats to insert the tire tool to lower the winch assembly. You can then reach under the van and pull the tire out.
