just behind the blower motor under the passanger side dash is a plug that is the risistor. two screws hold it in place. just remove the two screws and pull out. xterraparts.com has themm for about $19. Xterraparts.com has pictures of the part and a diagram. In the diagram, the front arrow is pointing to the cabin not the front of the car. Follow the four blue and white wires from the blower motor to the blower motor resistor.
Blows on #4 only? I would replace the resistor. #4 speed by-passes the resistor.
The blower motor speed resistor is probably burned out.
One can replace blower motor resistor on 2003 Nissan altima by removing the old using spanners and replacing it with a new one.

There is a clip that is depressed and then the resistor will slide out. Remove both ends of the resistor and then replace with the new resistor.
Check to see if you have 12 volts at the blower motor. If yes then the blower motor is bad. If no, then the resistor, relay, or fuse is bad.
