The United States is in North America.
Your B1/B2 visa is stamped into your passport. If it is no longer there, that means someone has torn out a page from your passport, or has stolen your passport. You need to report this to the US embassy, and to local authorities. You will be issued a new passport, then you can return to the embassy to have a new visa stamped.

Yes, the decision of not granting Schengen visa can be appealed. Unfortunately the process is not universal meaning that not every embassy or consulate follows the same procedures. Even the appeal period varies from country to country. Some countries provide 30 days visa appeal perido and some countries provide 60 days visa appeal period. In any case, whether you can or cannot appeal visa would be mentioned on the decision letter.
infinity
It depends on what the VISA was denied for. Example if your wife has HIV, you wouldn't appeal. Usually you may appeal the case.
It takes at least six months to get a C1D type of visa.
I think the only way is to get a letter from the place you will start to work that says you are working for them and you are an employee with insurance and a salary. I think you might not be able to get it from a gas station or likewise, but better ask them. But I know for a fact that it is illegal to work with a tourist visa, your tourist visa will be revoked, you will be deported and your employer will pay fines if caught.
