Most probable reason was incorrect adjustment at time of installation. Assuming all the correct parts were fitted and the parts were assembled correctly by the maufacturer then the only other problem could be the final adjustment. There must be clearance between the throw out bearing and the fingers of the pressure plate when the clutch is in the engaged (driving) position.

If you are asking what a clutch shaft is related to the installation of a clutch, it is a plastic alignment shaft that is used to line the clutch disc up with the pilot bearing in the flywheel, so that when you install the transmission the transmission input shaft will slide thru the clutch splines and into the pilot bearing in the flywheel with relative ease. They are a must for any clutch installation!

i went on eBay and bought one for 149$ clutch and pressure plate with alingment tool, but installation might cost you a couple bucks!
You can find a Stage 2 or 3 clutch on eBay for about the same price as you could buy and OME clutch at your local autoparts store. About 150 for the clutch, and 100 to 300 for installation (depending on the mechenic). -Antwon00

Rotate the switch on the rod for the master cylinder for the clutch. Take out the retaining clip. Remove the clutch pedal position switch from the rod. Disconnect the electrical plug. Plug the new clutch pedal position switch in and installation is the reverse of removal
It cost me about $120 for a lifetime warranty clutch from Autozone, and then the average estimate for installation is about $350. You could also do it yourself for just the price of the clutch and a few new tools, but that depends on how much of a grease monkey you are.
