I will be straight and say I am not Nissan familiar with the trucks they make.However the issue is pretty much the normal in terms of problems.First I will ask is the clutch and related parts such as the pilot bearing and release bearing(also known as the throw out) have alot of either age/wear/mileage on it.Normal symptoms of a throw out bearing noise are these,noisy in neutral with the clutch pedal NOT depressed,noisy while in gear(any gear),but the noise will go away when the pedal is pushed in.In most cases yes the throw out bearing be noisy under the stated issues would point to it being worn and should be replaced along with a complete clutch kit as a good rule of thumb.Now the other side of the issue is you can replace the clutch and related items and release bearing and if for some reason the noise is STILL present whether in neutral or in gear that would point to the bearings in the transmission itself.If this is the case you would need the transmission rebuilt at least to replace the bearings for the transmission.The only concern I have based on your question is the grinding sound to (ME) points to the bearings in the transmission.A release bearing can duplicate this noise and if your clutch is the original I would replace it and the parts suggested first.Unless you actually drain the manual transmission gear fluid and find alot of metal shavings on the drain plug that should have a magnet on it's tip it would point definetily to the bearings in the transmissions around the gears.I hope this helps give you some usefull info and while my area is Mazda brand the issue is like I said pretty much the same regardless of brand unless Nissan is prone to certain things over time that clearly I would not be aware of.OK
