Check all your hoses and connections. Leaky Radiator? Possible clog in line.
AnswerI just had this problem with a 95 mustang. It was a blocked up radiator. Unfortunately we found that out after getting the heads decked, new intake gaskets, new head gaskets, new water pump, flushing the block, and a new thermostat.
check the fins on the radiator if they are loose you need a new rad
Has the rad ever been flushed? Try that if there is no indication of water pump failure (leaking or venting coolant from pump.)

tighten hose clamp or replace gasket on thermostat housing The hole in the block next to the thermostat housing is the water pump weep hole. If coolant is coming out of that hole, the water pump has failed.
Check the water pump. If the thermostat is working properly then the next common problem would be a bad water pump.

the thermostat is next to the water pump. Follow the lower radiator hose from the rad to the engine. that is were you will find the thermostat.
I would be thinking it is a head gasket.
