Time to visit a trusted A/T specialist. You probably have way more problems that a DIY repair can solve. But then again, You did not just change the Transmission Fluid and not use Honda ATF? If so, that may be the problem. You must use genuine Honda ATF in this transmission or you will experience the symptoms you describe. Normally a D4 light flashing means the computer has detected a major fault. The fault code can be retried with a scanner.
If there is a loud noise when shifting from park into drive, it may be a transmission problem. One should shift slowly, as the transmission could disengage itself.

An automatic transmission needs no manual shifting - a continuously variable transmission needs no manual shifting. Based on those parameters I believe that you are okay.

If it vibrates briefly only while shifting, it's probably the transmission slipping. If all other gears are shifting ok, you're about to loose the transmission.

Are you sure it is the transmission that is clunking? It could be the u-joints on the drive shaft that are making the noise. When the u-joints on the drive shaft are bad, there is an audble clunk when you accelerate and when slowing down (basically the same motions you go through when shifting gears).
