If the distributor has been out of the truck recently, could be it wasn't reinstalled correctly. If the distributor has not been disturbed, sounds like a timing chain problem. ANOTHER ANSWER: Every time the rotor turns one complete round the crankshaft will turn twice. Knowing that, the crankshaft will pass TDC TWICE while the rotor will only point to the #1 cylinder once. If the crankshaft is at TDC and the rotor is not pointing at #1 it may just be that the rotor is pointing directly opposite. If that's the case, just turn the engine by hand, once around until it's pointing at TDC again.

first you need to get a diagram that shows you were number one plug wire goes, and then the rotor should be pointing at it when the motor is a tdc, but if it helps mie was facing the drives side.
I assume your working on a S.B. Chevy 350 - If so the rotor turns clockwise. turn the engine to TDC on the compression stroke, and wire the cap starting at the tower where the rotor it pointing. 18436572 is the firing order, and the drivers side bank is 1357 from the front, and the passenger side is 2468
With the # 1 piston at TDC on the compression stroke the rotor will be pointing at #1 plug wire location.

It really doesn't matter where it points as long as it's pointing at the post on the distributor that has the number one wire on it. Just remember, that's a 50/50 chance. It could be 180 degrees out of time. At TDC, the rotor should be pointing toward the position where the number 1 spark plug wire plugs into. If the rotor is pointing in the opposite direction you need to rotate the engine once more since the engine rotates twice, once on the compression stroke (TDC) and once on the exhaust stroke (not TDC).
once the number 1 piston is on tdc which ever point the rotor is facing becomes the number one position. if you conect the wires from there and follow your firing order you should have no problems.(i have done it)
It's not marked because #1 can be just about anywhere. When the engine comes from the factory, it's always the same, but if the distributor has been removed, #1 will be whereever the rotor is pointing when #1 piston is at TDC at the top of the compression cycle.
The easiest way to find it is to set #1 piston at TDC and see where the rotor is pointing, it should be pointing to number 1 terminal on the distributor cap.
