Not knowing what vehicle you have, many vehicles use vacuum to operate some, if not all, the HVAC (Heating, Ventilation, Air Conditioning) controls. If the vacuum supply is cut off for whatever reason the HVAC goes into defrost mode. I suspect you have a disconnected/broken/damaged/pinched vacuum supply line usually found under the hood leading from the engine into the firewall.

Most likely a bad or disconnected vacuum hose to the defrost/floor selector in the heater ducting.

There may be several reasons why the air only blows out the front vents and not on the floor or the defrost on the 2003 Dodge Ram. The fuses may be blown, or the heating coil is not working correctly.

Check under the hood for a broken or disconnected vacuum line.
Something like this just happened when my a/c blower was just changed. The mechanic forgot to plug in some of the wires around my blower which caused the air to blow out the dash and the floor only. It was three wires (one yellow, one white, and one red)that my husband pluged in. After that it worked fine! Just a suggestion!!!
The 1979 Chevrolet Corvette air flow door might the stock in the position so that air only blows out of the floor vents. The air flow door is located next to the blower fan.
The year, make and model info would help. Also need to know if the blower motor is working, just not getting any heat, or not changing function between defrost, dash or floor vents.
The blower might need replacing. Normal cause is a failed blower resistor, caused by a blower fan that is failing.The resistor and fan are located above the passenger side floor behind the glove box.

This is the same on all cars. When the air flow is OFF in a section, the it blows stronger to the other vents. In this case the ones for the windshield. This makes defrosting quicker so you can start driving safely sooner.
