Check the U-Joints in the drive lines. More times than not it is these that are really bad.
If there is a loud noise when shifting from park into drive, it may be a transmission problem. One should shift slowly, as the transmission could disengage itself.

I have a 1998 dodge dakota. I had a whirring noise that i thought was the transmission. Was real loud when i accellerated and shifted. it turned out to be the clutch on the fan that draws air thru the radiator was locked up and would make a real loud whirring noise because the clutch wasn't slipping as well as affected performance and shifting. I have a `00 blazer and recently my car started making a loud winding noise also. Come to find out I had to have a whole new transmission due to that noise! The repair shop said that the noise is the first sign of the transmission going out!
Worn motor and/or tranny mount.
When the mount is broke off the engine you will hear a loud clicking sound from then engine being unstable and shifting around. Listen close enough and you can hear it shifting when the vehicle jerks.
Loud and sustained or repetitive noise.
Internal transmission failure.
you may have to replace the transmission fluid and the screen but before that ask the technician to flush your transmission
