Can mean a few things. Low hydraulic fluid( brake fluid) in clutch master cylinder, worn clutch disc, needs bleed, slave cylinder slowly losing seal, seals in master cylinder, linkage problem. It could go forever but it is probably just the clutch disk material. To sum it up it means take it to the shop because if you don't know what it could be you most likely don't know how to fix it and it will only get worse to the point where it will not move
You will eventually lose clutch pedal pressure and the car will refuse to shift into gear.
The same as it mean everywhere else - that depressing the clutch pedal pressurizes a hydrualic fluid, and that pressure is then used to operate the actual clutch mechanism in the transmission.
I assume you mean the clutch pressure plate. If that is what you mean there may not be a cover. On older cars and trucks there is a flywheel cover on the bottom of the bell housing. At any rate to replace the clutch pressure plate you will have to remove the transmission. Then the bell housing to gain access to the clutch plate mounting bolts.

The driver applies or releases pressure on the clutch to move the car a little or to hold the car in one place on a hill. It's called slipping because the clutch plate slips against the flywheel. This is not good for the clutch.

The clutch will have to be replaced. The pressure plate could possibly be resurfaced but usually it is as cost effective to replace the pressure plate.
