There are 2 answers: the official and the generally accepted.

Official is: A transgender wants to have a physical change to closer mirror how they perceive themselves; hormones or surgery or both.

A transvestite is a person who wears the clothing of the opposite sex either to appear as if they are the opposite sex or for sexual fantasy. More often they are the former and not the latter.

Unofficial: A transgender is a person who perceives themselves as not fitting into the binary gender theory i.e. there are 2 genders, boy and girl, you are one or the other, not both, not one one time, not one another. They feel this forced structure is incorrect and harmful.

A transsexual person is already born as the opposite sex as far as who they are, but with a birth anomaly. Eg: a woman wrongly born with male parts or a man wrongly born with female parts who eventually gets corrective surgery to become physically who they have been all along. Thinking of a transsexual woman as a "man who becomes a woman" is quite degrading.

A transvestite is a man who wants to be and acts like a woman and also dresses like a woman and without hormones and surgery. The preferred term is a crossdresser, since transvestite can carry a clinical or sexual connotation.

A transgenderist is somewhere between a transsexual and a transsexual. They want to be like the other sex and be accepted as such to a certain degree, but without getting the surgery. Some of them may be confused and may refer to themselves as transsexuals, thus harming mainstream, gender conformist transsexuals by confusing others about what transsexualism is.

Transgendered is a political grouping and catch-all label for everyone who is gender-variant. Many actual transsexuals who seek to rid themselves of transsexualism with surgery and be gender-conforming refuse to be lumped under the TG or even the LGBT banner, and believe that is insulting, degrading, furthers causes they object to, robs them of rights, and destroys their reputations. The media and LGBT groups may think it is a softer way of saying transsexual, but it is not. Most under the TG grouping are gender non-conformists, don't want complete transition, and keep many of the beliefs, attitudes, and behaviors of their birth sex. They may be more sexually active than TSs, more likely to be left-wing in their politics, more likely to hate religious groups, and more likely to side with LGBTs and support gay issues at the expense of TSs. TSs tend to want the same things as other women, not more, less, nor different. Some TSs allege that being lumped with TGs destroys their chances of the mainstream, cisgendered acceptance that they need, sexualizes what they consider a birth defect, and robs them of their voice. Plenty of TSs complain that when in TG circles, they are expected to hide their true nature and keep their objections to themselves. Otherwise they face one-sided, unchecked ridicule, censorship, or even banishment from the very TG circles that claim to represent them.

Transwoman is short for "transgendered woman," and is often used incorrectly to include persons born with genuine transsexualism. It is a person who was born with male equipment but alleges they have a 'female mind' and wants to live as a woman, often without surgery. The disparity between how they want others to think of them and what they really are causes them to go lengths to try to change how society perceives them. If you ask these persons "Are you a girl or a boy?" they instantly (no hesitation) say "I am a girl." "but between your legs is a penis." "So what - in here (pointing to heart) I know I am a girl." A transwoman lives as a woman daily even if they are not one inside.

A transvestite is a man OR woman who dresses in the clothing of the opposite sex, often to appear as if they were that sex, BUT they do not wish to do any permanent changes to their bodies, they only wish to occasionally wear the clothes (and breasts and wigs and...) to appear as if they are of the opposite sex.

A transsexual person is a person born with the sex organs of one sex, but the neurology, spirit, soul, essence and being of the other sex. They usually consider themselves as having a birth defect, and most object to being called transgendered or transwomen. Transgender women are gender variant persons born with male bodies, who live as women, identify with the LGBT, and usually want to keep their genitalia of birth. A number of transsexuals get confused by the Propaganda which places them under the transgender umbrella and refer to themselves as transgendered persons or "trans(gender)women." Anyway, TS woman really are women, albeit with a birth defect, and they go to great lengths to destroy this birth defect with hormones and surgery, hoping to be treated exactly the same as women born without transsexualism and not considered transgendered in any way.

Most transsexual women have known from a very young age they were girls and not boys. They did not in anytime of their live "convince" themselves they were female. They already know they were female.

Updated information on this topic can be found in library and on the internet about transpeople and transsexuals. This subject is highly debated amongst the public, doctors, psychiatrists, scientists and so on.

First of all, it must be said that there are no absolute definitions for most gender terms such as these. Any difference comes from who is talking, and how they are using the words. So it is always best to use the terms of the other person and not try to interchange them or try to use euphemisms. If someone says they have transsexualism, it may be seen as rude to try to change the topic to discuss other things such as transgenderism, crossdressing, or homosexuality.

A Transsexual person is a person who was born into a body that contradicts who they are and needs surgery to correct this. The psychological establishment generally requires candidates for sex-reassignment to live as the opposite sex for a period of time until being able to have surgery. However, the psychological state of mind has been correct at the time of their birth. That means they don't change to something else, but make their bodies match who they have always been. Persons genuinely born with transsexualism are not a part of the transgendered community, and many transsexual persons find it offensive for others lump them into the transgender community or compare them with such persons. Their medical procedures are correctly called transsexual procedures or sex-reassignment surgery, not transgender procedures nor gender reassignment.

A Transgender person is any gender-variant person who doesn't generally get genital surgery (if MtfF). This includes cross-dressers, transvestites, transgenderists, drag queens, non-op porn stars, and genderqueer persons. This term describes anyone who lives or dresses like the other sex for non-therapeutic reasons, be it cross-dressing, or acting female or male. The term Transgender does not address the aspect of physically correcting the body. Some may lump transsexual persons into this category, while many transsexual persons refuse to be lumped into this category. Most transsexual persons are not part of the transgender community.

So putting it more simply, a transsexual woman is a woman born wrongly with male parts and desperately needs them gone, while a MtF transgendered person is a man who simply dresses or lives as a woman. Likewise, a transsexual man was wrongly born with female parts and needs a more masculine body, while a transgendered person born with female parts is simply a woman trying to live as a man to some degree.

Both are generic terms used by different groups. At different times they both meant a person who is born with genitals that are different from their sex. Others use the term gender to speak of how we perceive ourselves. The end result is, it means what the speaker means: it has to be discovered in context.

Some people use transgender to mean a person who perceives themselves differently in the social role or physical parts than most of society. That is, there is more than "boy" and "girl" .. that each of us has both within us and that at certain times we act more one way than another. This makes us more than the "gender binary" that is exclusive "either you is a boy or you is a girl, it's that simple." Except, it's not that simple.

Lately an explanation by some people is : sex hangs between your legs, gender hangs between your ears. In this case a transsexual wants surgery to match their sex to their gender. A transgender becomes a person who may or may not want a physical change, but who is definitely not part of the "normal" (ahem) sex definition, because their gender isn't "either-or."

A transsexual is someone who identifies as the polar opposite from their birth sex. Transgender can define SO MANY different gender identities such as intersex, genderfluid, androgynous, transman, transwoman, etc.

The difference between transgender and transsexual is that transsexuals often get surgery to change their sex while transgenders simply don't identify with the sex roles that are placed on them.
