Joseph Smith left New York for a couple reasons. First, there were so many new converts in Ohio that it was decided Church headquarters should move there. Second, there were many people in New York that were hunting Joseph down and he needed to escape.
A handful of Church members were sent from Ohio to settle near Independance, Missouri. Joseph Smith desired to build "Zion" in Jackson County, Missouri, and sent this group there to prepare the land.
Persecution against them became so bad in 1838, that the Saints left Ohio, some joining those in Missouri, and others moving to Nauvoo, Illinois. Illinois was chosen because the persecution in Missouri was also rising, and it wasn't certain if the Mormons could stay much longer.
Eventually, the Mormons in Missouri joined those in Nauvoo. After the murder of Joseph Smith, the Illinois government asked the Mormons to leave due to the controversy and mob violence that had increased since their arrival. They did so, and the first groups arrived in Utah in 1847.
