It shouldn't. My 94 4.3L runs smoothly at all rpms and all temps. I've had it over 9 years, so I've had a lot of time to notice something like that. i'd start checking for codes, then do a tune-up.

Agreed.. check for codes ..if none found do a tune up..plugs wires cap rotor.. and may want to replace coil to eliminate that as well.. there are number of other things it could be if that doesnt solve it..but it's cheapest and easiest to start with..
