The Montero has an automatic Idle Speed control. The idle is automatically set during operation by the vehicles engine control module. If the vehicle stays at the 2000 RPM idle, check vacuum lines for leaks, replace the fuel and air filter, and check the timing. All of these can cause the ecm to improperly compensate. However, it is normal for the Montero to 'high-idle' often, especially when using the air conditioner.

Only on cold startup (diesel engine) it can be normal to get little black smoke, as long as it doesn't continue. If it continues after warm up that could indicate engine wear. I'd recommend a mechanical inspection. The newer common rail diesels though are generally cleaner burning than older engines.

Of course it is not normal. You have no fuel pressure at startup, and the engine is starving for fuel. Look at the fuel pressure regulator or for leaking fuel injectors that are bleeding off at shut down. This is definitely not normal and anyone who tells you it is, should not be trusted to repair this vehicle.

The Caddy Seville? That is definitely not normal for that car to make a noise like that, maybe you should bring it in for inspection.
