Incorrect timing can cause several problems. One of which is high idling. 5000 rpm sounds high though, there may be other problems as well.
Yes, though they arent imediately obvious as they are hidden by a red tint. Turn on ignition so the dash lights up, but dont start the engine, put the gearstick in reverse and walk round back of the car. voila.
If the engine is running/idling to high, it will make putting it into reverse very difficult. Manual transmissions rarely have internal problems aside from clutch assembly replacement. That's not to say it couldn't have a problem though.
The Peel P50 is the world's smallest production car. About 100 mpg from the moped engine. Top speed of around 35 MPH. No Reverse gear though.

It is just the heat shield on the bottom of the cat that rattles. As to whether it is critical to proper vehicular function.. :/ Idk, I wouldn't think so. Might ask a real mechanic, not a shade tree mechanic though. Right now, I just let it rattle on my '01.
When my V6 fuel pump was inoperative the engine sputtered and made bad sounds like it had jumped time. It was the fuse (thousands of miles since) and was fixed in the parking lot quickly - had me worried for a few minutes though.
