Alright entering prestige mode in MW3 is pretty easy. You can only enter prestige by leveling up to 80. Once you reached level 80, go to the multiplayer menu, play online whether it be on the Playstation 3 or Xbox 360, go the icon where it says Barracks, once you go there Prestige mode should be ready for you.

If the disk is scratched or dirty it can keep the eye from being able to read the disk.
The disk might be faulty in the area the data is stored.
Disk might be worn out if it is played allot.
If the disk is dirty clean it with a microfiber cloth starting from the middle and wiping outward.
