DIET drinks, generally do not contain ANY sugar or calories(and often no caffine). Unfortunately because of this many companies use aspertame in their beverages for various reasons which has shown to cause cancer in MASS quantities. Your doctor would probally say "don't do it because we know it is bad." But at the same time, too much iron or salt will kill you too but with out either you'll also die, so drinking aspertame will likely NOT cause you cancer. On that note some diet drinks do contain salt too, often because it's needed to flavor the drink.
So, in the end 90% of the time diet drinks will have no negative effects on your body except for maybe staining your teeth (most soda is clear by default and contains food coloring).
Downside of diet drinks:
Often lack in preservatives, natural and otherwise thus shortening their shelf.
Altered taste because a ton of chemistry goes into the original beverage (generally much more than you can do on your stove at home) and removing some ingredients that make the drink on healthy make it just about impossible to flavor the drink while keeping it 100% healthy. (If you've had a diet drink you already know that the holy grail of soda is a diet drink that taste like the original!)
