The purpose of the EMG test primarily is to determine if there is a problem with a nerve or nerves, if there is a problem how bad that problem is and where that problem is. From this information interpretations and recommendations can be made that may include things like more testing, prescribing medication, physical therapy, bracing, injections, change in therapies, nutritional supplementation, &/or surgery or other medical procedures. You and your doctor may even decide to do nothing and just monitor your symptoms. An EMG measures the electrical activity within the muscle by use of a sterile needle electrode, and there are many characteristics that an electromyographer can look at when evaluating an EMG. The activity within the muscle is recorded by the EMG machine and represented graphically on an electromyogram. This electromyogram measures the electrical activity of motor unit potentials within the muscles and represents them graphically on an X-Y axis. The X axis of the electromyogram represents time measured in milliseconds, and the Y axis of the electromyogram represents the amplitude of the motor unit measured in voltage. Because these electrical responses are represented in waves, those waves can be converted to sounds and listened to by the electromyographer. The examiner observing this activity can then make interpretations of the activity while watching and listening to the electromyogram. Often this begins with listening to the activity of the muscle with the needle moving in the muscle (called insertional activity), listening and observing to the activity of the muscle while it is at rest, listening and observing the motor unit morphology and recruitment patterns of the muscle with gentle contraction, and finally listening and observing the interference pattern of the muscle when it is at full contraction.

After 30 years of being a workers comp patient I've had some exposure to the red tape involved in workers comp. In most cases your carrier will approve a EMG doctor that your primary w/c doctor sends you to for the test. Keep in mind that some adjusters may say no. Then your attorney is forced to file a special form so an umpire type entity can determine whats fair. This would be an exception rather than a rule for an EMG is not an expensive test. Your adjuster may change over and over as time passes. If your adjuster will talk to you (many cold ones ask you to contact them through your attorney) be nice even if you want to curse him or her out. The test results will be about the same. Like an EEG that you might get done for your heart and EMG will yield results that are what they are. For example lets say your a bit rude to the EMG doctor or tech. It want change the results of the EMG. Your main interest is how your nerves are conducting, not what opinion the EMG doctor has of you. Its like meauring your weight and height, they are what they are.

What would cause ulnar nerve damage not to show up on an EMG?
EMG's work on the same principle as an electrical wire and voltage transmission/measurement. Two probes are inserted, one at the top of the arm or the upper end of the nerve, the other one near the wrist before it goes past the wrist bone and into the pinky.
The upper probe is usually the transmission probe, the other the receiving. Since a specific, measurable amount of voltage is used, the voltage drop between probes can be measured and compared with established norms. Depending on the voltage drop between the probes, it will show how much actual nerve damage there is, or isn't. The reason no nerve damage is showing on the test is that there isn't enough voltage drop suggesting any real damage. If it were damaged, the drop would be measurable and comparable to established parameters. While it may have been slight, it obviously wasn't bad enough to be measurable.
Keep in mind that just because you have ulnar nerve pain doesn't mean it's damaged - in fact, if you have pain it means it's working well and something is compressing it elsewhere. For "elsewhere" I'd consider whether or not you've had any neck injuries, as arm nerves originate in the cervical spinal area. My own ulnar nerve constantly hurts due to injuries at the elbow, and major spinal damage in the neck which left permanent nerve damage.
