What does severe bilateral arthropathy in the lumbar 4 and 5 mean?

Picture a tripod, with the body of the vertebra as one leg, and the facet joints as the other two legs. Looking down on the spine from above with the stomach in the front... The body of the vertebra is at 12 o'clock (noon) and the facet joints are at 5 & 7 o'clock [posterior (behind) & lateral (off to the side)] of the main vertebral body.

Facet joints help stabilize the spine, and wear & tear over a long period of time will lead to the body strengthening these joints by depositing more bone to make them stronger. This is called arthritis.

Nerves exit the spinal cord and travel between these facet joints; when there is a lot of bone build-up, the causes crowding in the area where the nerves exit the spine to travel down the body. This narrowing is called 'stenosis'.

If there is enough 'stenosis' of this area the nerve exiting the spine in this area can have some pressure placed on it.

Nerves are like the wiring in the house, except individual nerves fibers are bundled up together, like a lot of individual small wires within the main bundle. Each individual wire (nerve fiber) travels to one specific spot in the brain, traveling all the way from its origin in the body, through the spinal column (cord) to the brain.

If this nerve fiber in affected ANYWHERE along its path, it will make the brain think pain is originating where the nerve fiber originated. So if a nerve bundle in "pinched" in the spine, the brain will get the signal that (let's say, assuming the nerve came from the foot) an area in the foot is hurting.

The Doctors at Shriners Hostpital say thyat adolescents do not want to wear a brace that makes them look and feel different from their friends.

The accepted treatment for adolescents with a curve of 20 to 40 degrees has been bracing, however, there is no hard data to support that it works.

If bracing is prescribed then the brace must be worn for the allocated time the Dr informs, which is usually 20-22 hours per day, this can be hard on a child, especially an adolescent

Is there any advice for scoliosis?

the person who has it should be understood, they will get very tired and sore,

dont make the person walk too much or carry too many heavy things but don't baby them,

have them see a doctor, but dont trust everything the doctors say, have them refer you to a specialist. who you can believe,

no matter how long it takes get the right treatment for them.

if they have to have a back brace, be sympathetic, they are uncomfortable as they train your spine into a different way of growing, they might also be insecure about wearing it to school.

if they have to have the operation, be prepared for a shock,

they will have to learn to walk again and undergo lots of physio. its also very uncomfortable to have rods in your back as you can feel them, this will be a big shock to that person,

they might have to be on strong medication for at least two months, it can be very hard to give up the medication when you are in that much pain, they could have sleeping problems and withdrawls you just have to support them through it the whole way, and dont give up

because when they come out of it the other end, they will see things in a new light and everything will be great :)

Who treats scoliosis?

You should always deal with a competent surgeon for scoliosis, your first point of call would be your GP/Dr who would then refer you to see a surgeon who will take x-rays and evaluate treatment

What is rotatory dextroscoliosis?

I am not a Doctor, but I do suffer from this condition. In layman's terms, the regular (usual) curvature associated with scoliosis is intensified by a rotation and/or a reversal of the curve, generally noticed with visible bone deformity in the ribcage parallel to the spine and noted in MRI with multiple disc disturbances in the thoracic/lumbar spine. It hurts. It is generally inoperable and does degenerate over time. Mine is compounded by degenerative disc disease and multifaceted arthropathy along with spinal stenosis, osteoarthritis and rheumatoid arthritis and fibromyalgia. I am a 34yr old male of Irish/Norwegian descent. I have no idea if that is relevant, but maybe an inquisitive team of doctors will read about it and contact me with some relief suggestions.

What are all the types of scoliosis?

Scoliosis curves are measured and expressed by degree. The higher the degree the more the scoliosis is said to be "severe".

The normal thoracolumbar spine is relatively straight in the sagittal plane and has a double curve in the coronal plane. The thoracic spine in convex posteriorly (kyphosis) and the lumbar spine is convex anteriorly (lordosis). Normally there should be no lateral curvature of the spine.

Double Major Curve: is difficult to identify as it causes only minor visual distortion. A double major curve is made up of two curves with nearly equal angles.

Double Thoracic Curve: For type 2 DT curves, the general rule is to fuse both the PT and MT regions posteriorly. The upper end vertebra of fusion is usually either T2 (left shoulder high before surgery) or T3 (shoulders level, or, rarely, right shoulder high before surgery).

Lumbar Major Curve: effects the lower part of the spine and often curves to the left.

Main Thoracic Curve: or type 1 MT curves, the general rule is to fuse only the MT region, posteriorly, anteriorly, or, if occasionally required, circumferentially.

Thoracolumbar Curve: is longer than most and extends from the upper to the lower spine. It may curve either to the right or the left.

Triple Major Curve: For type 4 TM curves, the general rule is to fuse all three regions. Either the MT or TL/L region can be the major curve, but all three regions are structural.

Adult Scoliosis - Scoliosis is discovered after the patient has stopped growing
Degenerative Scoliosis - The adult who has pre-existing scoliosis will show signs of wear and tear or osteoarthritis (can happen to anyone as we age) - but because of the deformity, their spine starts to collapse and their curve can progress. The other type of person has no scoliosis to start with, but as he or she ages, the spine starts to show signs of wear and tear and it collapses asymmetrically. These individuals actually develop scoliosis later in life, even though they didn't have a pre-existing condition.
Neuromuscular Scoliosis - is due to loss of control of the nerves or muscles that support the spine. The most common causes of this type of Scoliosis are cerebral palsy & muscular dystrophy.

What does mild dextroscoliosis may be positional?

To answer this, it would be easiest to break the word dextroscoliosis into its parts. Dextro comes from the Latin word for "right", and scoliosis is curvature of the spine. Given this information, mild dextroscoliosis refers to a minor curvature of the spine toward the right side of the body.

They are shaped essentially like a hocky puck and are filled with a thick material.

If the disk becomes worn out in an area it will bulge out, like a bubble. If the bubble [herniated area] bulges against a nerve about to leave the spine you'll get a lot of burning pain along where the nerve runs [like down the back of the leg with sciatica].

Roughly 70% of the herniated disks heal themselves within a few short weeks and scar in place, rarely to bother you again.

Current research has suggested that a bulging disk does not necessarily cause pain by compressing nerves, but more likely by sensitizing nerves through local inflammation factors released by damaged cells and cells of the immune system (cytokines). Thus, over time as the inflammation goes down the pain will also go away. Usually, even after the pain is gone the disk bulge or herniation will remain, sometimes contacting nerves or even the spinal cord. It has been suggested that as many as 30% of the population has one or more asymptomatic (non-painful) disk bulges (1-4). It is likely a normal part of aging, so unless it is causing you pain, don't panic :)
