In most women the hymen doesn't cover much of the vaginal opening - often it's nothing more than a narrow cresent shaped piece of tissue on the back of the vaginal opening, thus often at most all that happens is that the tampon gets sutck behind the hymen and it just takes re-angling to remove the tampon.

If the hymen covers a little more of the vaginal opening then yes the expanded tampon may stretch the hymen - the hymen stretching is perfectly normal, it shouldn't cause any pain and doesn't mean anything.
