It should not. it would be like becoming a warewolf. The people fear you but a lv4 vampire does not get attacked. the vampire lord should not effect gameplay except that removing dawnguard may cause problems. the vampire lord is weakened by the sun but you can still get the well rested bonus.

What is elder scrolls 5 Skyrim about?
The return of the dragons (as predicted by the Elder Scrolls), as well as the rise of a Nord god. You play as the last Dovahkiin (Dragonborn in the tongue of the dragons), who (quite obviously) will stop this threat.
Skyrim is an open-world game which lets the player follow his own path in the world. There is no real obligation to finish the main quest, and you can continue playing afterwards anyway.
