You say 'places' like you are talking about some far away land, but remember that many in rich countries are living in poverty or homeless so they can't afford tampons, pads, or cups either.

In less economically developed countries many women simply bleed on clothing or use items like old rags or newspaper, in some parts of Africa or India girls have to drop out of school once they start menstruating as it's not possible for them to attend without menstrual products. You have a lot of charities and menstrual product companies that are working on schemes to tackle that problem. There's also Menstrual Hygiene Day about raising awareness about this issue.

In developed countries, for example United Kingdom or United States of America, there are many people who can't afford menstrual products because they live in poverty or are homeless. In many cases they will have to choose between food or menstrual supplies, they will steal menstrual products, or like in less economically developed countries they will use newspaper or rags.

Some women's shelters may have menstrual products but often this will only happen if there is a drive for them (as they're not commonly donated otherwise). There is also a campaign to get menstrual products for free in public bathrooms, this is to help all women but would also be a great advantage to those who are poor or homeless as it would give them access too - it's called Free The Tampons.
