You must access the bolt that holds the spare tire in place from the inside of the car. There is a flap in the carpet in the rear storage space behind the last row of seats. Lift the flap, loosen the bolt, and the spare tire will be lowered so that you can remove it.
It is possible that your Sienna does not have a spare. Mine was equipped with roll flat tires (RFT) which eliminate the need (in Toyota's mind) for a spare. Saves them a few cubic feet which they can then tout in advertising. i have a 2004 totoya sienna and the spare is under the passinger rear slider door the carpet has a little flap right behind the passinger seat when u open the little flap or carpet there a nut that u turn and the spare will start too come down underneath

Lift the rear door, Under the mat on the right side there is a little flap cut into the carpet. Under the flap there is a nut you put your tire wrench on and rotate with the wrench to lower it

I had the same problem and solved it by simply going up a guy who had the same truck and borrowed his mechanism. Or you can go to a dealer and get them to lower the spare. I now have the spare in the back. I plan to to hire a welder to make a bracket, to hold the spare outside the vehicle. I find the Toyota system impractical. On a cold, wet dark night on the highway, you don't want to be messing with finding that hole
There is a push button that can be pressed up or down (it is rectangular in shape) located below the air vent to the left of the steering column.

We encountered this piece of "fun" when a tire blew in a road trip. It took us, 4 other people and two State Troopers (we got stuck on a freeway where it says No Stopping) over an HOUR to find this. Used vehicle, no manual. So - behind the front seats, move the 2nd row of seats back, roll up the carpeting. Under there is a small depression with a lugnut. The wheel spanner fits this and you crack the spare down ... check for progress and obstructions while you lower the wheel.
