If it's the original transmitter and it has gone out of sync with the vehicle, hold down both the unlock and lock buttons until the car horn honks. This should re-sync. If it's a replacement transmitter, I think the dealer will have to program it.
You cannot reprogram it. A dealer can get you the Master Code if you've lost it.
You go to the dealership. Take proof of car ownership(registration) and proof of ID (Drivers license) it doesn't cost that much and it can be done there at the dealership in just a few minutes. Open the keyless remote and you will see the battery take it out and replace it with the same size.

This works for the 1993 Grand Cherokee: Sit inside the vehicle with all the doors closed. Place ignition in the OFF position. Open the driver's door. Leave Open. Move the mechanical door lever to the LOCK position. Turn the ignition to the RUN position. Aim the transmitter at the Keyless Entry receiver and press Lock button. Hold for five seconds until the door locks cycle. Turn ignition OFF within 20 seconds.
