Complete failure of the tensioner may well throw the valve timing out enough for the pistons struck the valves. I'm not an expert on Audis but I'd certainly seek advice before just putting a new tensioner and belt on.
At 72,000 miles - but it is vital to replace things like the tensioner pulley, idler pulley, water pump as well. My cambelt was replaced but not the pulleys. The tensioner pulley bearings failed at 120,000 miles which allowed the cambelt to slip and wrecked the engine. Not economic to repair so the car (worth approx £2,500) was a constructive write-off plus cost of stripping down to investigate, hire cars etc, etc. Complete disaster. Currently looking to garage that serviced it to reimburse me.

When does it happen? Is it constant? What engine? How loud? how long does it last? Had the same thing in my Ram Truck found it to be the water pump. Possible failed belt tensioner.
yes
Possible causes include a failed cam sensor, broken wiring, broken timing belt, failed computer.Possible causes include a failed cam sensor, broken wiring, broken timing belt, failed computer.
Ran out of gas, failed fuel pump, water in the gas. Alternator, crank sensor, cam sensor, computer, coil, broken wiring. You don't. It employs a spring loaded automatic belt tensioner pulley located usually on the passenger side of the engine. If your serpentine belt is too loose, either the tensioner pulley has failed (possible, but unlikely) or the belt has stretched beyond the limits of the tensioner pulley and needs to be replaced (more likely).
Your Explorer has a serpentine belt tensioner which keeps the proper tension on the serpentine belt unless the tensioner has failed.

The fan is electric, not belt driven. The serpentine belt is has a spring loaded tensioner. If the belt is loose, the tensioner has failed.
