The main cause for Islamophobia is not the consistent acts of terrorism performed by Muslims. [These include the 9/11 bombings which called hundreds of Americans and also the 7/7 bombings in London as well as many others.] Some feel that due to the fact that Muslims performed these bombings, this has led to the illusion that all Muslims are bombers and that they should be feared. Exaggeration in the media of Muslims has not led to Islamophobia. Instead, the media, and U.S. government have fanned the flames of something that is older than 9/11.

Islamophobia reaches far back in history, and deep within the Western psyche. Exploration and colonization of the Islamic world brought with it a wave of Propaganda that characterized Muslims as inferior, primitive, violent beings. This was part of the political justification for invading and occupying these lands. These deep-rooted misconceptions are part of what is being used to inflame people against Islam and Muslims.

Bigotry against minorities is not new. Fear of the 'other' is not new. What is new is the security concern that is now attached to Islamophobia. Reporting in the media focuses on the 'bad Muslims' and the sensational nature of much of modern media exacerbates the issue. Government actions, such as bending the laws of privacy and due process, serve to shore up people's fears, adding to the intensity of Islamophobia.

Issues with the term Islamophobia
Islamophobia is a nonsense term. The term is defined as "a hatred or fear of Islam or Muslims", lumping in criticism and fear of the religion of Islam and its political pieces (which is entirely legitimate) with the hatred or fear of individual people, which is discrimination. People are allowed to hate ideas; not all ideas are good ideas and worthy of respect. Any criticism of Islam or of Muslim practices just gets branded with this label, even if the words are spoken by Muslims. Therefore, I propose (in this answer) three separate words to discuss the varied feelings of hatred towards Muslims, religious Islam, and political Islam:

  • Anti-Mohammedan(ism) or Anti-Muslim Bigotry = The belief that Muslims as people are wicked or problematic. This is the Muslim rough equivalent to Anti-Semitism for Jews. The term actually existed historically, but has been "outmoded" because Muslims are no longer called Mohammedans in recognition of the fact that they do not follow Mohammed because he is their Lord and Savior, but only as a Prophet.
  • Anti-Islamic(ism) = The belief that the ideology and beliefs of Islam as a religious construction and lifestyle manifesto promotes wrong-doing or are seriously problematic to a multicultural society.
  • Anti-Islam(ism) = The belief that Islam as politically manifest and capable of determining actual laws promotes inequality, injustice, and moral incorrectness.

What causes Islamophobia?
Quite simply, Anti-Islamism comes from seeing what Islamic States look like. The usefully named "Islamic State", and numerous Muslim-majority countries are ones with horrible penal codes, human rights deficiencies, and other issues which result directly from a literalist implementation of a number of Islamic doctrines. Anti-Islamicism is less common and usually comes from a rejection of Islam as that person's choice of religion (or Atheism) and the disagreement with certain views that are famous in the religion's choice of prayer. Anti-Muslim Bigotry usually derives from fear that a significant number of Muslims are Islamists or fundamentalist in their outlook and is stoked when left-wing individuals and Muslim community leaders play the "there's nothing to see here" game. There is a nexus between Islam and Islamism. Concordantly, it is far more worth it to get agitated over whether we have found the people who made the ideological jump and stop them from acting, than to try and divert attention from the problem.

Contrary to the community answer, the reason that Anti-Muslim Bigotry is much higher than Anti-Hindu Bigotry is explicitly because of the violent actions of Jihadists (violent Islamists). While racism and othering are certainly part of Anti-Muslim Bigotry, to discount Jihadist violence shows either an ignorance or intentional disregard of what so-called Islamophobes actually say.
