When a turtle starts to swell up, there could be many issues that need to be attended to. For example, the turtle could have an abscess. It could have shell rot or internal parasites.

There are a few things that could cause this, first, was the food changed? water turtles can be especially picky, if you change food you would have to do it gradually. Does the turtle have a heat lamp? They are cold blooded and would need to bask or at least have the light on during the hours of daylight, this gives them vitamins that regular light can not produce. Nothing major is needed, just a small lamp and a 60w "heat lamp" bulb, (the bulb is important you can't use a regular bulb) The other major thing is that turtles have to feel secure when they are eating, they like fake plants, you don't need alot of them but some around where you feed them so they feel safe. I've read that it was a shock to put them in colder water when they were just in warm water. All you have to do is simply replace the water with the same temperature as the original water.

Turtles normally bury themselves deep in the mud and hibernate all winter. Obviously, a captive turtle in a tank cannot do this. It can be very dangerous to let a pet turtle hibernate at home.

Normally, people get a UV heat lamp for their turtles. The UV heat lamp simulates the summer sun, so the turtle goes through the winter without hibernating. The heat lamp can be left up all year round, if you want.

Heat lamps can be purchased at Petco and other pet stores. You can also buy a timer for the lamp, so you don't have to be home to turn it on every morning, and off every night.
