I charge $15 per walk in MA. It really depends on where you live and what the going rate/time is. In affluent communities it's more than blue collar or "normal" working class areas. Call around to other dog walkers and ask them what their rates are. It's called shopping the competition.

You should charge for every walk, yes. That doesn't mean you shouldn't offer discounts for plans which involve purchasing more walks than 1 per day. For example you might offer a 10% discount for 2 walks per day. For your infrequent clients, you might consider offering them bulk plans where they purchase 10 walks at a time up front. For your regular weekly clients charge them a week in advance rather than per walk. Many walkers charge in arears, I do not, I learned charging ahead of time works much better in terms of getting paid in a timely fashion. You might also consider using recurrent credit card billing via PayPal.
Yes, it is not unreasonable to charge a surcharge for "out of control" dogs. If you are a capable trainer, you can offer retraining or behavior modification to your services.
