Why haven't the antibiotics helped a sinus and ear infection?
The sinus membranes are semipermeable and only certain ABs can penetrate them.
(I took Ceclor in 1985 for a winter sinus infection and it worked, but a friend had a serious reaction to it on his fifth dose and I took him to the ER for treatment which included steroids and antihistamines and the warning to avoid the drug forever. At that time some 30-plus people had died from Ceclor, but BIG PHARMA has so much political influence that the bods have to really pile up to get an approved drug taken off the market)
So, you can't just take some leftover ABs from the medicine cabinet for a sinus infection. Most ABs are ineffective for sinus infections. Go to the doctor and ask for something, preferably something other than Ceclor.
Ear infections require great care and that usually means ABs in the form of eardrops (possibly in conjunction with oral meds) and (one thing that's often overlooked is) great care should be taken to prevent any bath, shower, river, lake or pool water from entering the ears. If water gets in your ears you should carefully dry them with cotton swabs to avoid problems. (the level of germs in municipal or well water can fluctuate depending on many factors and even "good water" supplies can have high levels of bacteria at certain times)
All water has microbes in it, and if one is prone to ear infections then water in the ears should be completely avoided. If you swim in lakes or pools (or go cave diving) then a peroxide solution should be used after ward and that's if you have no problem with ear infections. Bubble bath soap should never enter the ear canal, and unless the tub is sterilized with industrial high pressure steam cleaning equipment I never submerge my ears in a tub. (Germs thrive in tubs even if the tubs are gleaming and appear to be clean)
It's ill advised to plug the ears before swimming because the pressure must be equalized if your head is submerged, so if you are prone to ear infections it's best not to swim. Better to splash around in shallow water and always keep your head completely dry.
What do you do for ear pain and muffled hearing caused by itchy ear in the middle of the night?
In the middle of the night I work up with my ear itching. After pushing a million times on the cartilage covering the hole to my ear, I awoke in the morning to pain and muffled hearing. As a medical student I took a deep breathe from the crying and the pain and thought about what was occurring. I believe my ear was irritated whether from wax, particles, etc. I irritated the canal as well as the membranes. My ear didn't hurt to the touch but was sore on the inside.
I took 2 ibuprofen for the swelling.
Mixed 1 part water to 3 parts Hydrogen Peroxide (3%) on the bottle and heated it in the microwave for 20 seconds. Put it in my ear, then let it out. The warmth helped loosen any wax.
Then I put Hydrogen Peroxide (without water) into my ear and let it sit for 10 minutes, while laying down on the opposite side.
I poured it out into a tissue and lay ed on my sore ear to allow the remaining Hydrogen Peroxide to drain.
Although this process will have to be completed for the next couple days, I can honestly say that I went from having a pain level of 8 to a 1. There is still some popping in my ear and it is still a little muffled but I will continue to take ibuprofen for the next couple days and use Hydrogen Peroxide in the mornings.
*Be careful not to use Hydrogen Peroxide for a long period of time, you can dry out your eardrum.
If you get a cut or a open wound, you can get dirt in there.

Will giving a pug yogurt help with a yeast ear infection?
If your pug has a yeast ear infection you need to take it to the vet for antibiotics and medicated ear drops. It is sometimes recommended as an additional home remedy for human babies with thrush in their mouths, but I would not suggest this at all for pugs. You need to take your pug to the doctor!
-Yogurt will help a yeast infection. Antibiotics will only work and be needed if there is a bacterial infection. Chronic ear infections in pugs (I have 2) are usually actually an allergy to the grains in their food which is also something to explore with your vet. We do grain free food and yogurt everyday.-

Fresh Water however (not chlorinated as in a public bath) might expose you to various bacteria that ultimately might cause an ear infection or other infectuary problems.
Statistics can be used in order to determine the likelihood of an infectious disease spreading from a particular place.
If a doctor get many people with similar conditions after swimming in the same place/water, then this increases the likelihood of this.
