Why do people in some places eat using knife and fork?
Some cultures eat with their hands, some with chop sticks. Where knife and fork are used, it could be said, simply: that is just how things are. But, there is also more to it than that--
As to cultural origins, eating with utensils is, no doubt, an exercise in class distinction. Using utensils separates the people with (at least a modicum of) social standing (class) from those without. Among those having higher levels of social standing, wealth and class distinctions are further put on display by use of utensils made with varying degrees of crafted ornateness and of more or less content and value of precious metals.
Another reason -- and this reason also touches on the question as to why Asian emigrants to the west do not more readily, or in larger numbers, adopt utensils over chop sticks -- is that knife and fork are simply the most efficient solid-food eating implements over all alternatives (including chopsticks, including hands). Because efficiency (in contrast to oriental reverence for tradition and for one's elders) is and long has been regarded highly in the west, it is there that fork and knife are and have long since been the standard utensils for eating.

Should you eat pizza with knife and fork?
if it really hot and has a lot of toppings so that it will make a big mess, then yes, but, normally, just eat it with your hands if possible. but, make sure to wash them first- well most of the time.
Another common way to eat a pizza is to partially fold the slice. One has to take care doing this if it is hot or has a lot of toppings, since the toppings and cheese can spill over that way. In that case, get a plate and utensils, and either eat it with utensils or at least eat over a plate and use the utensils to eat or replace what falls off.
I was indeed Saturday Night Live and I think it was in the 1980s. It was some kind of Candid Camera parody which took place in a diner. They showed the fork being left on the burner and an old lady eating cheesecake. I am thinking it would have been done during the Christopher Guest years as the show went from being live to having more filmed comedy.
I only saw it once and I was quite young. I was searching for it today as it should be on streaming Netflix.
Best I can do.
Yes it was a parody with "Allen Funt" The Hot fork trick!
I even remember the skit dialog ! lol
(allen funt - whispering in the background..."this poor lady thinks she biting into some "cool -creamy- cheesecake"...wont she be surprised when -all she gets is a mouth full of seared flesh!!! (allen funt laughing .
the old lady bites in...freaks out in pain ..almost falls off the diner stool.... and if you watch close the fork actually STICKS to the roof of her mouth as she reacts !!! LOLOLOL way funny ! good times... good times !
