How can you obtain a Certificate of Live Birth?
A Certificate of Live Birth is the same thing as a birth certificate. These are produced by the states, unless it is a certificate of live birth that is obtained from an official of a foreign government as proof of the US citizenship of a newborn when a US citizen gives birth outside the US. Some states will also provide this as a shortened version of another document called the long form birth certificate. It just use it to means the same thing in different words. If you contact your state's office of vital records they can likely tell you the process to get a copy of the "long form" if all you have is the short version and you need the additional detail or they can give you the information on how to obtain one.

When the copy machine was invented it became common practice to simply copy the souvenir hospital certificate and certify that, thus also making a legal certified birth certificate. Now in the computer age when everything is computerized it is common practice to print out a form with the necessary information and certify that. All of them are valid, legal, certified birth certificates except the original hospital certificate which has not been certified by the issuing state.

it is basically the same throughout the state. contact the police department that covers your municipality (either town police or state police). they will give you a form which you need to fill out (need to be 18 years of age or older), and a form that basically needs your signature allowing them to search your mental health records. return these forms to the police with a $5 check or money order. then you need to make an appointment with Sagem Morpho for fingerprinting (costs about $60). return a copy of the fingerprinting form and the recipt they give you to the police. then wait until they contact you for the card pickup.
What if I have been using a wrong social security number?
If you are the victim in a report and your ssn is wrong, you will want to contact the police department that wrote the report and have the report corrected and reprinted. If you do not need the report for an insurance or other legal reason, I wouldn't worry too much, but, if you are the perpetrator or guilty party, I would disregard the error. If the authorities want you, they either know where to find you or they cannot find you without searching by your ssn. Then, I imagine it would be a good break for you and a mistake on the part of the PD. They will wish they had paid better attention to what they were doing.
