If you can remember back to the young Cloud flashback in disc 1 (Just after Midgar), it's in the mansion where Sephiroth was looking at all the books, you actually walked past where Vincent is/was back then (but obviously couldn't get to him).
Go up the stairs in the Shinra mansion (located in Nibelheim), there on the left side in the northmost room is the safe.

... do you mean Xion? If so, Xion isn't Kairi's nobody, Namine is. Xion is simply the result of an experiment in Castle Oblivion on Sora; she's an "imperfect replica of Sora". That's why she's able to wield the keyblade

Why cant you get the ability scan for Ursula?
I don't know if this is what you're wondering, but I could have sworn you got the ability "scan" earlier in the game than ursula for KH1. I think for every battle it is small and with Atlantica it blends in. In KH2, it is a musical and of course you cannot get the "scan" ability. In KH1, I got "scan" before defeating Sabor in Deep Jungle.
