That is a very good question! Sore throat is very common in the society. Most of the time it heals without any treatment. It is easy to teach, but difficult to fallow. You have to take treatment for ten days. Everybody including the contributor fails to comply. Once you become well, you tend to forget the doses of penicillin. So you have about 15 million cases of rheumatic heart disease and every year about 300,00 people die of the same number of patients join in. When the patient comes to you with rheumatic fever, you have to put him on long term benzathine penicillin prophylaxis. After few months, some quack tells him that there is no need to take monthly injection and patient stops the same. Same story is repeated more than once, before the patient can be convinced. If the facility is not available, then you have to use Tablet penicillin for sore throat. tablet penicillin 250 mg three time a day for ten days, empirically.
