Fiji, the 54th country, was suspended following a military coup in 2006.

3 countries have left the Commonwealth: Ireland, left 18 April 1948, Zimbabwe, left 7 December 2003, and South Africa, left on 31 May 1961 and rejoined on 1 June 1994.

I know of at least three; Massachusetts, Kentucky, Virginia.

Pennsylvania also refers to itself as a Commonwealth.

Pennylvania is also a Commonwealth State.
