The incoming cable needs to go to a modem that converts the incoming DSL Symbols to a digital data stream. This is then passed to the router in the form of some sort of LAN (Local Area Net) format like Ethernet.
There may be combined modem/router devices around, but I am not aware of any.
Note...I used the term "DSL symbols" because the symbol concept was originally a DSL trick, but it is now used in other non-DSL systems. DSL stands for Digital Subscriber Loop...this gets deeper and deeper as I talk about it.

Houses generally run on single phase power. It would not make sense to convert single phase to three phase to run a house. Please restate the question.

The process of obtaining 220 volts from 380 volts is quite simple. 380 volts is a three phase four wire system voltage. The 220 volts is obtained by taking the sq. root of 3 which equals 1.73 and dividing it into the phase voltage. 380/1.73 = 220 volts. This holds true with any three phase four wire voltage system. 208/1.73 = 120 volts, 415/1.73 = 230 volts, 480/1.73 = 277 volts and 600/1.73 = 347 volts. This lower voltage is present on any of the three phase legs of the three phase system to the neutral which is grounded on a wye connection.

The voltage insulation rating is not the factor that cables need to determine how much current that a wire can carry. Most home wiring is rated at 300 volts. A 3C #10 cable is rated for 30 amps. This is the cable that is usually used on dryer connections.

A transformer has a rating that is usually expressed in KVA. This is approximately a wattage rating. It is not dangerous but it can be the cause of some concern. An appliance has a set current that is draws. This current times the voltage is the appliance's wattage. The same goes for the transformer. It only has a certain capacity to supply a specific current that is governed by its KVA (watts). Driving the transformer beyond its rated capacity tends to heat the transformer beyond its working temperature. If left in this over current draw the transformer's windings insulation will break down and the windings will short circuit. This is usually the end of a working transformer. So short answer, more watts (amps) from appliance equals burned out transformer.

This action was done in the past but not so much now. It was a case of the manufacturer installing a grounding pin in their cords while homes were still using two blade receptacles. Prior to new homes having to install a grounding receptacle, homes were wired with only two slot receptacles. These two slot receptacles just had a "hot" and neutral connection for 120 volt output supply and no accommodation for the grounding of the receptacle. How can you include phase failure relay in your star delta motor starter circuit?
The star delta motor starter circuit has to have a control voltage to energize the motor magnetic contractors. A phase failure relay monitors all three phases of the incoming supply lines. The phase failure relay also has auxiliary contacts on it. There are usually two sets of "C" form contacts. The control voltage for the starter circuit must be connected across the normally open (N.O.) contacts of one of these two sets. When the phase monitor is energized these N.O. contacts close supplying the control voltage to the circuitry of the star delta starters. If there is a phase interruption, phase reversal or phase loss, the phase failure relay will de-energize and the N.O. contacts will go back to their de-energized state. If the star delta motor starter is in use at the time, its control circuitry voltage will be lost and the delta motor magnetic contactor will open. The opening of this contactor will disconnect the motor from the failed three phase supply.
