It's just something you can do. You are allowed to poke friends and people. If you press the button, they are notified of it. It may be kind of a way to get somebody's attention. I know some of my friends have "poke wars" where they just poke each other back and forth. So nothing much special happens, they will just be notified that you "poked" them.

Simply save the picture in My Pictures as a PNG or JPG when you save your finished image. You can do this by typing in your image title when the save screen comes up and adding '.png' or '.jpg' to the end. Then go to facebook.com and upload the picture as you would normally.
