Hip Replacement

In a total hip replacement, damaged hips are replaced with artificial hip parts called prosthesis, a device that substitutes or supplements a joint. Questions and answers are invited about the surgery and the technology involved in this procedure.

When was the first artificial hip replaced?

In 1891 T.Gluck wrote about attempts in Germany to replace the hip ball using ivory to replace the femoral head.

On September 28, 1940 at Columbia Hospital in Columbia, South Carolina, Dr. Austin T. Moore, performed the first metallic hip replacement surgery.

Why would the leg and ankle be swollen after hip replacement?

Though there could be multiple reasons behind this, and a real-time examination of the patient is required to make a proper diagnosis, a probable cause could be the compression of the venous network of the hip region involved in the surgery, due to which the venous return of the ankles and legs, which is primarily via the great saphenous vein draining into femoral vein, gets interrupted, causing pooling of blood and lymph in the lower limb, causing edema and hence swollen leg and ankle.

What is the ICD-9 code for hip arthroplasty?

996.78 OTHER COMPLICATIONS OF INTERNAL PROSTHETIC DEVICE

PX IS 78.67 REMOVAL OF IMPLANTED DEVICES FROM BONE

FROM AHA CODING CLINIC

This is fine if you have a prosthetic device. However, if you just have scar tissue (arthrofibrosis) you would code 718.57

Why is pure iron not good for a hip replacement?

Iron will corrode (rust) in the body for a start. It is also not strong enough, the surface would also wear quickly.

How long after hip replacement should pain go away?

If an elbow replacement is any indicator you dont see much joy for about 3 weeks.

Can you use a hoyer lift after hip replacement?

You CAN use a hoyer lift after a hip replacement. You have to be mindful of the patient's hip precautions during the transfer. Using a pillow between the patient's legs will help to avoid excessive hip adduction. When possible use multiple people to assist in the transfer. Be sure that the patient's hip flexion does not exceed 90 degrees.

Depending on the reason for the transfer there may be better options than a hoyer lift. If it is for the purpose of getting a patient out of bed into a chair a stand pivot transfer using a gait belt bed>chair would be a good option. Also a slide board transfer into a stretcher chair would also be a better option. Simply sitting on the edge of the bed (with supervision) for an extended period of time would have similar benefits to sitting in a chair.

To sum it up you can use a hoyer lift for hip replacement patients however there are better options. I have personally only used a hoyer lift with hip replacement patients that have fallen out of bed and there is no other lift options. Any kind of transfer is safe as long as you adhere to the total hip precautions (no flexion > 90 degrees, no crossing the legs, no internal rotation) and weight bearing status. Have a qualified person assist you with transfers (occupational and physical therapy) if they are available.

What exercises can you do to lose weight two years after hip replacement?

Consult with your Doctor. In many cases the low impact provided by exercise in a pool can be beneficial.
Every case is different, so double check with your doctor, but most exercises are OK, though I would avoid ones that involve a great deal of bending over.

If you are worried, a great one would be swimming at it doesn't put too much stress on your hip.

What is a hip replacement?

Exactly what it sounds like. A hip bone is taken out, through surgery of course, and then is replaced with a prostetic. My mom's 'new' hip is made of titanium.

A bone conserving alternative to a Total Hip Replacement (THR) is hip resurfacing. The Birmingham Hip Resurfacing (BHR) is a bone conserving implant aimed at young active patients who want there quality of life back!

Instead of replacing the old worn out joint, a BHR resurfaces the femoral head with a Cobalt-Chrome component and inserts a Cobalt-Chrome cup into the acetabular socket. This metal on metal technique last longer and has less wear than the traditional Metal on Plastic THR.

By conserving the bone at the femoral head, revisions should they be needed are much simplier than they are if a THR has been carried out at first.

The BHR was pioneered by Derek McMinn in Birmingham, UK. The implant has a high success rate currently at 97% at 13 years which compared to similar statistics for a THR, the difference is staggering. See McMinn Centre website for more details!

What is the success rate for hip replacement?

It is estimated that one in seven adults over the age of 60 will have knee replacement surgery. Those who are extremely active in competitive sports such as football, hockey or other contact sports can have a higher instance of needing this type of operation. The procedures have become minimally invasive with a recovery time of about five to ten days.

Can a person run after a hip replacement?

You can do anything you want to do. As long as you don't have an underlying bone problem such as osteoporosis. They are a godsend for these patients who've dealt with bad arthritis for years. Most people will be able to do much more than they could before the surgery.

Ankle swelling after hip replacement surgery?

This depends, as in most individual cases, upon the treatment the physician provides both before the actual birth and during the gestation stage. Some swell, some do not. Some women approach the hospital wearing large pillows on their feet and some came running barefooted. My feet are swollen this very minute because you have forced me to be up and about long past my bedtime.

Layperson's Answer:

I have experienced such swelling after surgery, hands (I couldn't make a fist), feet, even face (the surgery was abdominal). My face was so swollen, I had no peripheral vision. I believe that this was due to the amount of IV fluids that they infused following my surgery. When I spoke to the physician about it, he would not say one way or the other. I know that they do need to provide these fluids following surgery but I also believe that they keep them flowing for their own convenience, to administer medications into the IV line and so that they don't need to monitor the patient's intake of fluids. Just my opinion.

What is safest hip replacement implant?

All invasive surgery carries hazard, as is any anaesthetic. This operation is usually performed on elderly folk, who have increased risk because of their age.

But it has a high success rate to an otherwise disabling condition.

What age was the Oldest person to have hip replacement?

When I was having mine done there was a 91 year old lady having one done. UK.

My Dad just had his hip replaced, and he is 6 weeks shy of 100 years old. Still active, has all his mental faculties, lives in his own home with my Mom who is 95. Indiana

Is a Birmingham hip replacement the same as a DePuy hip replacement?

Both companies have products for hip resurfacing and for total hip replacement as well. Birmingham is owned by Smith & Nephew, and Depuy is owned by Johnson & Johnson. A correct comparison would be between their hip resurfacing systems, the BHR (Birmingham Hip Resurfacing) and ASR (Articular Surface Replacement) from Depuy and they are certainly not the same, although it can be said that they are similar, both are metal on metal implants.

In rates of failures the "ASR system has a failure rate of 12% at 5 years compared to 4.3% of the BHR" - according to a study.

Also a crucial component of the ASR system, the ASR XL Acetabular System has been recalled in 2010 due to high failure rates.

Are there alternates for hip replacement?

One alternative to a traditional total hip replacement is Hip Resurfacing. The Birmingham Hip Resurfacing (BHR) was pioneered in 1997 by Derek McMinn. The BHR currently has success rates as high as 97% after 13 years which is incredible compared to the statistics of total hip replacements.

The BHR works by resurfacing the Femoral head and fitting a metal, not plastic as used in Total Hip Replacements, acetabular component (socket). This Metal on Metal implant has significantly lower wear than Metal on Plastic which is so commonly used.

It is a bone conserving surgical procedure, which means in the future should a patient require revision surgery this is much easier than if the original procedure was a total hip replacement. There is no reason why patients should wait until they hit 60-65 before having a total hip replacement - if the hip is giving you pain and discomfort then resurfacing could be you. Many local doctors and GPs will tell you to sit tight and wait until you are a suitable patient for a total hip replacement. The BHR has FDA approval in the US and has over 13 years of analysed data showing high success rates!

The BHR is ideal for young active patients, who wish to get back to their previous active lifestyle. See the McMinn Centre website for more details!

What are the pros and cons of anterior and posterior hip replacements?

Posterior hip replacement is easier to perform (the majority of cases use this approach), however some studies indicate that it has a slightly higher risk off dislocation (but not significantly so).
Anterior approach is more technical (and requires more training), and may require special operating tables (which have been linked to complications such as broken ankles), however the recovery time is shorter (by a few days), and has a slightly lower risk of dislocation than the posterior approach.

Unless the surgeon is a regular user of the anterior approach (done 20+), I would go for the posterior approach.
