Assuming a business letter to husband and wife with different last names who you don't know the proper business salutation is to have the the wife's name first before the husband's accompanied by the proper titles.

For example:

Dear Mrs. Thompson and Mr. Jones, (salutation on letter)

Mrs. Susan Thompson and Mr. Jeremy Jones (address on envelope and letter)

Note that you could use "Ms." instead of "Mrs." if you prefer, although I recommend that since in this example you know they are married I would choose "Mrs."

Outside of the traditional, formal "Mr. & Mrs. John Doe", the wife's name is ALWAYS first when using first names: "Jane and John Doe". In social importance, the woman is always first, then males, then children. Traditionally, the man's first and surnames are never separated. The confused idea of the man's name first (John and Jane Doe or Mr. John Doe and Ms. Jane Smith) is neither traditional nor appropriate. At Emilypost.com, she notes that traditionally, a man's name was first on an envelope adddress (Mr. and Mrs. John Doe), and his first and surname were not separated (Jane and John Doe). Miss Manners' Guide to Excruciatingly Correct Behavior and Crane's Blue Book of Social Stationery both either state outright or give examples consistent with the following: Married Couples- formally- Mr. & Mrs. John Doe; informally- Jane and John Doe In which woman kept maiden name- Ms. Jane Smith and Mr. John Doe In which man is a doctor- Dr. & Mrs. John Doe In which woman is a doctor- Mr. and Mrs. John Doe or Dr. Jane Doe and Mr. John Doe In which both spouses are doctors -Dr. and Mrs. John Doe, or The Doctors Doe, or Dr. Jane Doe and Dr. John Doe

When writing to a female that you don't know, use the title Ms (or Ms.), a title invented just for the purpose of addressing a female when her marital status is irrelevant or unknown. The title 'Madam' (always capitalize a title when addressing someone) is still acceptable as well.

The proper way to address a Doctor of Osteopathic Medicine (DO) (not Doctor of Osteopathy) is Doctor (Dr.) since they are medical physicians.

The usual syntax is [name], [rank] (Ret'd). For example Dave Smith, Lieutenant (Ret'd)

Its a variable. It depends on how tall each are. To find the difference measure them, take the lesser from the greater.

Mrs. is when a women is married. Ms. is when they are not married. Miss is when thay are younger.

When using the noun "Brother" as a noun of address, it is a proper noun as a word for the specific person you are addressing.

The noun "brother" is a common noun as a general word for any male sibling.

"To Mr. and Dr. Smith."

Or just separate the names, "To Mr. John Smith and Dr. Mary Smith."

If it is a casual letter you could simply write "To The Smiths"

I am guessing the original form of address you want to modify is "To Dr. and Mrs. John A Smith." But, this is an increasingly archaic form of address and is not applicable in the case of the wife being the doctor or professor nor can it be used when the wife has not taken her husband's name.
