Assuming you mean at the shrine of Deepscorn Hollow, first you need to buy the Ichor of Sithis from the same person you bought the location's upgrades from. You will fill the basin and activate the shrine. After that, you can be healed from it, like from a regular shrine, but only if your Infamy is higher than your Fame.

there is a fighters guild in every cityexcept for in the imperial city. the easiest to find would be in Anvil. fast travel to the Anvil Main Gate and straight in front of you, past the tree, will be both a fighters guild and a mage's guild. the mage's guild always has a sign shaped like an eye above it and the fighters has a sign shaped like swords i think. another easy way to spot them is the fighters guild has red tapestry hung down the walls and the mage's guild blue.

There is no set location at Level 5 it will be random loot from dungeons and at Level 6 it will be sold from vendors. Best dungeons to look for them in are Necromancer and Conjurer dungeons.

Using the console command ~ you can alter many aspects of the game. Although this particular cheat is not going to mess you up... You still must use caution when using the console commands. You can easily mess up a future quest and be stuck in the game later on.

The command advskill <skill> <nn> will give you a skill level up and advlevel will give you a level up. Another useful console command is completequest <questid> for when you just want to get past a quest and move on.
