And no, you can't usually tell at birth. Transgender people are almost all born with normal-looking genitalia, and nobody does an MRI at birth, which might tell us something (some transgender people might differ in brain structure from cisgender people of the same apparent sex).

There are several methods for a man to develop natural breasts. the 1st is to take female hormones typically estrogen and progesterone, along with a testosterone blocker such as spirotone (estrogen is very weak after fighting with testosterone). Also some estrogen promoting chemicals found in certain plants called phytoestrogens help. The most important thing for this to be effective is to be young. ideally in early puberty. The stem cells that grow breasts tend to gradually dye out as you age. Of course if you are interested in plastic surgery there are always breast implants.

Progesterone cream and phytoestrogen pills may be purchased at retail stores, such as wal-mart.

Estradiol Valerate and Spirotone (aka Spirolactone) are available via pharmacies, with a prescription if in person, without a prescription online.

To fully transition takes at generally takes several years. Once a person makes the decision to fully transition from one gender to the other, they begin therapy and then when they are ready they start what is called a "real life test" where they are required to live as their "target" sex for one year. So a Male to Female TS woman will live and work as a woman for at least one year prior to being approved for the reassignment surgery.

SRS need not be contemplated for a licensed doctor (M.D.) to prescribe hormones.

Outside the USA it may differ. Within some asian countries obtaining prescription strength hormones is possible without prescription but for safety one should still use a doctor as too large a dose can have detrimental effects. IOW...more isn't always better.

Transformation effects can take up to and over a year although some effects will begin sooner.
