If you are playing on Hardcore any companion can be killed, Rex included. Otherwise, companions will simply be knocked unconscious and can't be killed. If you never finish the quest Nothin But a Hound Dog, I believe Rex dies as part of the end game sequence. I've never not finished that quest though, so I'm not sure.

no. the bb gun will do little to no damage to the super mutant. what i did is i sold my bb gun to moira in megaton with the ammo and sold some other stuff an i got a pretty decent weapon

She is in the fire station at camp searchlight. The door is locked and requires a key, you can get this key by killing all the prospectors in the camp searchlight storage which is located to the west of it.

There is a camera glitch you can use to get out; and if you have a companion with you firing them will cause them to open the door themselves.
