Yes, it surely can. There are many nerves in the spine (all organs are also connected to the spine) so you could have a pinched nerve. I would see your doctor and have that bump check out. Nothing terrible, but if it is causing you this problem it's best (if possible) to have it removed.

If you mean that it makes a hissing sound WHEN you breathe, then the yawning mechanism in that ear is probably forced open. Try to yawn hard and swallow often (not meant to sound sus)
However, if it's with every beat of your heart, that's just turbulence in your carotids and/or jugulars, nothing to worry about.

This kitten is bleeding internally and in great pain!
