You loosen it with a long 'breaker bar', without power tools I don't know of any other way.
If you have clearance and parts like the radiator and shroud out of the way, you can position the breaker bar on the bolt with the breaker bar laying over on the drivers side up against the frame then with the coil wire removed jog the engine over with the key and the bolt will break loose. I've done this in a pinch.

Yes, there are 2 or 3 similar tools, not the same quality, but nowhere near Fein's price either. - I have the Ryobi version, I only use it about once amonth and it does what I need. If I needed one for everyday use, I would definitely buy the Fein.
