When we see a person with vitiligo we must determine the evolutionary phase. We divide it in 5 phases (rapidly worsening, slowly worsening, stable for less than 1 year, stable for more than 1 year, and improving spontaneously).

If it is rapidly worsening there is a good indication to use oral medications. If it is slowly worsening it is usually unnecessary except if the hands and/or the feet are involved. In these areas it is very important to stop the progression as soon as possible.

It is very important to remember that the worsening phase lasts about 1 year (from 6 months to 1 year and a half). When you use a medication to stop the appearance of new spots it doesn't interrupt the phase. If you stop the medication before the phase fades away new spots can appear.

The death of melanocytes is progressive. Treating the vitiligo as soon as possible you turn the repigmentation faster because you will still have melanocytes in epidermis and in the hairs.

It is a steroid and steroids can certainly be used in vitiligo. They however offer temporary relief and cannot be used for prolonged periods due to their side effects. Betamethasone moreover is not the best steroid for vitiligo. You need something more potent like clobestol.

Some safe treatments are also available for vitiligo like PUVA, antiviligo.com, and recouleur.

What to do about red blotchy skin tone?

Nobody has perfect, airbrushed skin. Most fair coloured women have an ivory, red or yellow tone to their skin. Most dark coloured women have a yellow/golden, red, blue, olive or ashen/grey tone to their skin.

To help reduce your skin undertones, try using a concealer. Concealer does not just come in your natural shade. For best coverage use a concealer that is opposite your skin undertone colour on the colour wheel. For example, for dark circles under the eyes, use lilac. For blue veins, use peach.

To help reduce the red blotchy look, use a green concealer. You don't need to spend a lot of money on one. You can purchase one from somewhere like tescos. superdrug, etc. Use a small brush to apply a small amount upon the red patches. Once you've applied your concealer, apply a foundation that matches your skin exactly. To test a foundation dab a small amount on to your jaw line. If you test it on your hand then you will not get the right colour as your hands and face are two completely different colours as skin on your hand is a lot thicker.

DAB the foundation on using a spnge (do not slide as this will take off the concealer).

This should leave you with a good, flawless look.

Diet plays an important role in the treatment of Vitiligo. Cold and phlegm producing foods are harmful in this disease. Not only the response of the treatment is delayed but also the lesions can spread if such foods are used frequently. Food items which are restricted, and those recommended, are those given below:

TO BE USED:

Cereal: Whole grain food, millet/barley porridge, oats, rice, pasta (processed grains not

recommended)

Breads: only whole grain breads, bran wheat chapatti,

Fruits: Blue berries (people have different opinions), rasp berries, pears, cherries, banana,

pineapple, apple, mangos, apricots, grapes, papaya, amaranth

Yes, Indian people can have freckles. I personally have them, and so does my mother. It's a hereditary condition, and not many do. I have only met 3 other Indians with freckles, and they too have a more Caucasian skin tone.
