Can birth control be ineffective while taking metronidazole?

As far as less-permanent methods go, the "pill" (taken exactly as prescribed) is probably the most effective, followed very closely by the new injections and IUDs. It should go without saying but proper, careful and consistent use of ANY contraceptive method is the key to effectiveness.

You mean knowing whether you were still on birth control when falling pregnant? NO WAY. It is impossible to know so relax. If you work out when you conceived you can tellifyou were on birth control. However if you mean how do you know you are pregnant if you are using birth control, it is unlikely, it should rreally be called conception control, however if you think you may be, take a home test.

No. You can only miscarry in the third trimester. Taking birth control pills at this stage is unlikely to have any effect on you.
