This is a two part question:

1. How often should oxygen be used. Home oxygen therapy is used for patients unable to maintain an oxygen saturation over 88%. It is also used for people who have sleep apnea but do not need or tolerate CPAP therapy. If you are prescribed oxygen for sleep apnea, you only need to wear it at night. If you wear it for hypoxia (COPD, CHF, etc), you may need to only wear oxygen when you are active, or you may need it all the time. The precribing provider should give you that information. Follow the instructions your provider gave you.


2. Fluid restriction can be used to help control Congestive heart failure, in addition to diuretics. There is no strict number that works for everyone. You should discuss this with your provider. Typically, fluid restriction is less than 2 liters per day, but your provider will tailor this to your specific needs.

Tourniquets are dangerous when used improperly, such as having them applied too long, as they restrict blood flow to the extremity or limb, therefore resulting in failure of that limb or extremity.

The Initial, or Primary, Survery is a rapid evaluation to quickly identify life or limb threatening injury.

The Primary survey consists of ABCDE:

  1. Airway - evaluation of the airway includes ensuring the airway is unobstructed, the trachea is midline, and if necessary, securing the airway either with a supraglottic device, endotracheal intubation, or if necessary, cricothyrotomy/tracheostomy
  2. Breathing - ensuring breathing is normal. This includes observing the breathing pattern, effectiveness, and listening to the lungs. One should also observe the chest for abnormal mechanics
  3. Circulation - look for any obvious sources of hemorrhage and apply pressure, if necessary. Also, look at overall perfusion.
  4. Disability - a quick mental status evalution. Adult Trauma Life Support (ATLS) recommends using the AVPU mnemonic - Alert, responds to Verbal stimuli, responds to Painful stimuli, or Unresponsive.
  5. Exposure - remove all clothing, rapid anterior and posterior examination, and cover with warm blankets.

The primary survey can usually be completed in a matter of a few seconds if a patient is well appearing.
