Who had the authority to send people in to concentration camps?
The entire Nazi terror apparatus, including sections of the police, had the authority to send people to concentration camps. For example, sometimes the police decided that people with previous convictions were repeat offenders and, instead of sending them for trial, simply sent them to concentration camps.

Margot Frank is buried in Amsterdam along with her little sister Anne.
-No, the gravestone is in Bergen-Belsen, and no one knows for sure where their bodies are. The gravestone is merely a marker for the concentration camp where she died, along with her sister, Margot, within weeks of each other, at Bergen-Belsen. Both died of typhus. Anne died two weeks before the camp was liberated.
David who?! David Flair? He retired from wrestling a long time ago. Another David perhaps?!
How did the Germans make the Jews live in ghettos?
In towns in Nazi-occupied Poland and some other countries, an area was designated as the ghetto. All Jews were ordered into that area by a certain date, and all new-Jews were ordered to leave. The ghetto was enclosed by high walls and the perimeter was patrolled.
Some Jews in big cities knew that the ghetto was a death-trap and fled if they could. On the whole, though, the Jews did as they were told.
What was the population of Jews in 1919 in Germany?
There were more than half a million (religious) Jews in Germany in the years between 1919-1930 or about 0.75% of the whole German population. (Even if you add the non-religious Jews, half-Jews and quarter Jews the total is not more than about 800,000). 80% of German Jews were well educated and urban based.
Some had achieved prominence in politics, the press, business, banking and in universities.

You can find rosters of people who worked in CCC Camps in western Pennsylvania around 1928 to 1933 through the National Archives. This organization has a wide range of recorded documents concerning the Civilian Conservation Corps and other groups that existed throughout the history of the United States.
When did the final soloution actually begin?
It began before it was called the 'Final Solution'. So whereas Himmler may have only 'announced' the Final Solution on 4th October 1943, the mass murder started in June 1941 and the experiments in mass gassing started even before this.
______
One must never make the mistake of applying modern morality to the events of history.
Actually, the Final Solutions as The Final Solution was adopted at the Wannsee Conference that met on 20 January 1942 and implemented later that year. Prior to the conference, Hitler directed the attendees to discover a final solution to the "Jewish question." (...die Endlösung der Judenfrage).
Discussions on this topic are a minefield, but in the most clinical terms: Germany was fighting a war on two fronts. They also had a sizable population imprisoned in several countries. While mass executions had occurred, had actually been a matter of military policy, since the beginning of the war, especially in Poland and the Ukraine, they were not party policy. They became party policy after the Wannsee Conference. Not only did the Nazi government have to struggle to feed the civilian population, they had to fight to feed and equip the military so they could continue to fight the war. There was little left over for inmate populations. This is by no means an attempt to humanize an inhumane act, but the Nazi party made what they believed to be a practical decision and began executing mass numbers of Jews, Gypsies, homosexual, Jehovah's Witnesses, and the other groups who were in the camps, partially in what they believed to be an attempt to alleviate the strain on resources. In reality, Hitler and the Nazi's were devoted students of history. Most of the camp system and the events that ensued during the reign of the Nazis was modeled after similar programs that had been adopted by other governments in other nations. Hitler himself referred to the concentration camps originally as reservations. The Final Solution was in part modeled after the Armenian Holocaust of 1915-1917. And, Hitler's reference to the "Jewish Question" is an obvious homage to the political debate that existed in common thought after the Berlin Conference of 1878 and referred to as the Armenian Question. More goes on here the the demon actions of the Nazis.
____
With respect, the expression the Jewish Question has been in use in English since about 1750 (without any murderous intention and without the kind of obesessiveness that the Nazis gave it). It referred simply to the role of the Jews and such questions as whether judgments made by Jews on British soil in accordance with Jewish laws were actually binding and so on. There are, as far as I'm aware, only two references by Hitler to the Armenians and both are in unreliable sources ...
As for the actual Holocaust (Final Solution) it was already under way by the time that the Wannsee Conference took place. In fact, most of the Serbian Jews were dead by the end of 1941. The idea that the Holocaust (Final Solution) began with the Conference was abandoned by most specialists on the the subject three decades ago.

How and why was Lodz ghetto formed?
It was formed because the Nazis wanted to put all of the Jews from Lodz and the surrounding areas into one place. It was formed by the Nazis taking one area of Lodz and putting barbed wire fences and huge stone walls around it, to make sure that no one came in or got out. The Jews in the Lodz Ghetto had to work for the Nazis in order to get food.

What were eichmann's thoughts on Jews?
Like almost all Nazis, Adolf Eichmann bought into the idea that Jews were an inferior race. There was a pre-existing basis for this, called in anthropology Distrust of the Other and in the specific application of the Third Reich, anti-Semitism. However, the sadistic cruelty with which we associate Hitler and the Nazi extermination policies was the produce of a carefully-orchestrated process of gradually depriving the Jews of their civil rights and blaming them for their own misfortunes.
Immediately after the War, the Allies tried to rationalize the Germans were somehow uniquely prone to this kind of hatred, but in truth most of the victors had strong anti-Semitic attitudes of their own.
Fundamentally, Eichmann was not so much a savage hater as he was a self-important bureaucrat who was given an assignment to kill people and tired to please his masters by being very good at it.
Psychological experiments by researchers like Milgram and Zimbardo (look them up) demonstrated that it was surprisingly easy to get people to do cruel things, so Eichmann's deeds were, sadly, nothing very special or strange to the human mind.
