Some of these things include 1) what you are seen for (ear ache, chest pain, etc.) because the more things that need to be done for you make the visit more expensive. 2) what supplies are used on you during the visit. Even a bandaid is going to cost you! 3) What tests are done. A vist for something as simple as a sore throat or ear ache can cost $200. I've seen visits cost over $5000 for someone with a life threatening problem. An emergency visit can be quite costly so if you have a problem that can be taken care of by your regular doctor (the doctor you go to for shots and checkups) it may benefit you financially to do so.
