This depends on what state you live in. In Missouri you need to fill out a form which includes asking you your age, education, and job and any other names you are known under. They also ask for copies of divorce decrees if you have been married before. There is a fee but I can't remember what it is. In Missouri no blood test is needed but your have to apply for the certificate at least 3 days before the wedding and it is only good for 30 days.
